To enroll your child/children at Danville Local Schools you will need to fill out the following forms:  Application, Emergency Medical Form, and Free/Reduced Lunch.  These forms are linked below, and can be viewed by any PDF viewer software (Adobe, Foxit Reader, Preview, etc.).  Additionally, you will need to bring the following documents:  Proof of Residence, Birth Certificate, Social Security Card, Immunization Record, and any applicable Custody Documents.  Examples of acceptable Proof of Residence documents are included in the Application.
